Triangle face from memory, Revalations Pyramid head though. I kind of forgot how his helmet went so his helmet is more Homecoming-esque. 

But the weapon is the great cleaver which is only seen in the movie. As well as those chains that were stuck to him in the carousel seen. 

Also his legs are longer than a normal humans but have you seen PH in the movies he is tall as hell daaaaaaaamn [the actor's wearing very tall boots but if you think about it if that's a real monster with a pyramid helmet then daaaamn]
